In times of increasingly strained resources for healthcare providers, institutions need to use all means necessary to make care more efficient, improve patient outcomes and reduce the stress of its staff.
Dartford and Gravesham, a hospital trust in North Kent, U.K., has shown how our remote-monitoring wearable Current can help do just that. The NHS trust services 500,000 people at four sites. Its Hospital at Home team is tasked with monitoring patients after they are discharged from the hospital for a period of one or two weeks. The team makes frequent visits to patients’ homes to check vital signs and generally monitor patients’ well-being.
CIO Neil Perry wanted to see if the trust could reduce the number of visits needed without jeopardizing patients’ recovery, and turned to Current for help. In addition to reducing the need for manpower, Perry wanted real-time notification when a patient’s condition showed signs of deterioration — allowing for an earlier intervention and, ideally, preventing the need for rehospitalization.
It turned out to be a success, and once at-home patients were fitted with Current, unnecessary home visits reduced by 22 percent. In addition, the Hospital at Home team was able to visit higher-priority patients earlier.
“With Current, we’ve seen the ability to deliver intervention at a far earlier point and prevent readmission to the hospital,” Perry said.
Additional benefits included:
- Patients reported feeling more safe and comfortable using the product — which is crucial, because without that comfort patients are far less likely to use a device
- Current had a 92 percent adherence rate among patients during this trial, according to Dartford and Gravesham, much higher than the 50 percent rate reported by other products in other studies
- Dartford and Gravesham can better manage its resources, prioritize patients that need the most urgent care, and demonstrably improve patient outcomes
