How to Build Remote Care Programs Across Integrated Care Settings

As part of the NHS response to COVID-19, there has been a very rapid shift from in-person to remote care. This transition has realised many benefits including the delivery of more integrated and patient-centric care.

Norfolk and Norwich University Hospitals NHS Trust, share their experience of growing their remote care programme to a system-wide, 24/7 service supporting the delivery of care to multiple patient populations.

We also get a first look at Norwich’s plans to expand the reach of this service via a virtual hospital, which shall smooth patient transitions across their integrated care system and support the recovery of services.
The clinical leaders from Norwich also discuss:

  • The importance of integrating pharmacy services into remote care pathways
  • The impact of virtual care on patient and staff wellbeing, including one patient’s personal account
  • Outcomes achieved so far, including bed day savings, patient satisfaction improvements, and readmission reductions
